PRIVACY COLLECTION STATEMENT

Moore Rehab Outcomes is an independent practice under the ownership of Moore Rehab Outcomes Pty Limited.  You may contact Moore Rehab Outcomes by writing to: PO Box 4303 Shellharbour NSW 2529, by emailing admin@moorerehab.com.au or by calling 0435 947 334. You have the right to gain access to the information held by Moore Rehab Outcomes about you.

Our Privacy Policy (available upon request) contains information on how you may request access to, and correction of, your personal information and how you may complain about a breach of your privacy and how we will deal with such a complaint.

Moore Rehab Outcomes needs to collect information about you for the primary purpose of providing quality treatment. In order to fully assess, diagnose and treat you, we need to collect some personal information from you. This information will also be used for the administrative purposes of running the practice such as invoicing you, your insurer or compensation agency. Information will be used within the practice for handover if another practitioner provides you with assistance.

Moore Rehab Outcomes may disclose information regarding diagnosis or treatment to your Doctor or other treatment providers only with your consent. In the case of insurance or compensation claims, it may be necessary to disclose information and / or collect information that affects your treatment and return to work. Moore Rehab Outcomes will not disclose your information to commercial companies; however specific service or product information as deemed suitable for your management (for example to make a referral for agreed treatment program or to arrange care service or equipment delivery), may require us to provide your contact details.  This will always be discussed with you prior information is provided to another party. Your written consent will be obtained at the start of your treatment in order to carry out the above activities. We do not disclose your personal information to overseas recipients.

Information at Moore Rehab Outcomes is stored securely and only practice staff will have access to it. Moore Rehab Outcomes takes all reasonable steps to ensure that information collected about you is accurate, complete and up-to-date. You may have access to your information on request and if you believe that any of the information is inaccurate, we may be able to amend it accordingly.

If you do not provide relevant personal or health information, in part or in full, to Moore Rehab Outcomes it may result in incomplete assessment. This could impact on the diagnosis and any following therapy that is provided. MOORE REHAB OUTCOMES PRIVACY POLICY

  1. Information Collected

Moore Rehab Outcomes (MRO) is an independent practice under the ownership of Moore Rehab Outcomes Pty Limited. Information collected by MRO Clinicians should be fair, lawful and non-intrusive and is to be used to assist your recovery and rehabilitation.  You should receive a business card at time of your Initial Assessment with the MRO Clinicians name, phone number and email address and our MRO office number.

  1. Use & Disclosure

Moore Rehab Outcomes needs to collect information about you for the primary purpose of providing quality treatment. Moore Rehab Outcomes may disclose information regarding diagnosis or treatment to your Doctor or other treatment providers only with your consent. In the case of insurance or compensation claims, it may be necessary to disclose information and / or collect information that relates to your treatment and return to work. Moore Rehab Outcomes will not disclose your information to commercial companies and will always confirm with you regarding arrangements for equipment delivery, care service coordination or treatment referrals which may require release of your phone number or delivery address. Your written consent will be obtained at the start of your treatment in order to carry out the above activities.

There may be some instances where it is required by law or procedure to collect particular information.

Moore Rehab Outcomes takes all reasonable steps to ensure that information collected about you is accurate, complete and up-to-date.  We are often relying on you to provide accurate information and request that you are honest.

Moore Rehab Outcomes takes all reasonable steps to ensure that information collected about you is accurate, complete and up-to-date. This data is stored on hard copy (log on system which is password protected) and soft copy (files held by the Rehabilitation Clinician)

  1. Openness

Moore Rehab Outcomes is open with regards to the company’s privacy policy, a copy of this document is available on the website (www.moorerehab.com.au).  Information is also provided to the client at time of initial assessment so they are aware of how we obtain information.

  1. Access and Correction

You may have access to your information on request and if you believe that any of the information is inaccurate, we may be able to amend it accordingly. If you do not provide relevant personal or health information, in part or in full, to Moore Rehab Outcomes it may result in an incomplete assessment. This may impact on the diagnosis and the following therapy that is provided.

MRO are required to contact the funding body (e.g. Insurer) to notify them of your request for information and to get their approval to release this data.

All staff are trained in the procedures on how to respond to the request of information.

  1. Identifiers

The MRO system generates its own identifiers for each file, although your name, claim number and other personal details are used on all documents for purpose of identity.

  1. Anonymity

Wherever possible, MRO Consultants strive to maintain our clients’ anonymity in all conversations and contexts.

  1. Transborder Dataflow

We do not disclose your personal information to overseas recipients.

  1. Sensitive Information

MRO are often required to collect personal and sometimes sensitive information on our clients regarding their health and symptoms. MRO Clinicians are trained in maintaining confidentiality and treat this sensitive information with respect and only disclose necessary information to other parties (as per individual signed consent permission).
